  1. Critical Issue: What is one of the most important issue you're facing in your organization today? This problematic issue or barrier may include issues in design­ing and/or imple­ment­ing individual, group, or organiza­tional change.


  2. Difficult to Discuss: What is or has been difficult to discuss openly about the issue you describe in #1, e.g., what are your inner thoughts that you have not expressed verbally or in writing to the individual(s) involved? These individuals may be inside or outside your organization.


  3. Consequences of discussing the difficult underlying issue(s): From your inner thoughts, what do you see as the consequences (positive and negative, if any) of openly discussing the underlying issue(s) with the other party or parties?


  4. Consequences of NOT openly discussing the issue(s) and resolving it: Briefly identify the consequences of not being able to productively discuss it with the relevant stakeholders, such as:
